Usually Friction/preasure. some have been found to use a mild adhesive as well. a simple light glue should due the trick. I like white glue as it is easy to remove and the bond is secure without being overly strong. usually though I try to make sure that I can get a solid friction-only fit. I only use glue when that proves impossible.
you don't need a knife to remove it, your fingers pushing from the underside should be enough. if you encounter any real resistence, then there might be some adhesive that you'll need to remove.
To expand on what Gabe said. Take the bezel off the watch case. You should be able to push the insert out of the bezel from the back side. If it is glued in, you can get it out using a watch crystal press.
The 6005 Rally divers and skx007 bezel inserts I've removed have been both friction fit and clear double sided tape. First I remove the bezel using an exact o knife to lift the bezel off the case. Then I use a very thin case back opener and gently work it between the bezel and insert, moving around from underneath.

Be carefull: If you try to stick a knife under the bezel at 2:00 or 7:00 o'clock positions, you might damage the spring ring underneath the bezel (the one that make it rotate unidirectionaly).
